Untitled -> TODO: When did hospice service start?

2018-12-25

Bozenna and Jan with us for Christmas.

2018-12-27 -> TODO: Arrival, departure times.  (image/png)Bozenna went in, and brought juice with her. At 11:10am, the patient was thirsty, asking for water, so Bozenna asked for thickener to add to juice. The person on duty told her she would bring it with lunch, in about an hour. Bozenna asked for the thickener immediately, as her mother was thirsty. This did not happen, so Bozenna went to the supervisor to request the thickener. The supervisor [ … ] Bozenna told the supervisor that if nobody would provide thickener, she would call 911 to report a medical emergency.  -> TODO: What happened?

Bozenna requested a copy of the patient’s medical records.

Additionally, on this day, the hospice nurse finally came. She said the patient needs to be evaluated for swallowing.  -> TODO: What is the name of the nurse? -> TODO: What else did she say?

2018-12-31 -> TODO: Arrival, departure times.

When Bozenna arrives, the patient is lying flat (not in a reclined position), on an oxygen ventilator. The patient was struggling to cough.

Bozenna asked what had happened, and why her mother was on oxygen. The supervisor said that she had conducted a swallowing test by administering yogurt, and that as a result of this the patient had started to choke on the yogurt. No previous evaluation for swallowing had taken place between 2018-12-27 and 2018-12-31.  -> DOC: Is this in the charts?

Bozenna received the medical records. They contained an inaccurate account of 2018-12-27. The chart said that Bozenna had asked for water for her mother (not thickener), and that she had threatened to call the police, not 911. When Bozenna asked to have the chart collected  -> DOC: Get the charts.

Next up: Look into advance directive, DNR

-> Advice to Bozenna: must be polite, even friendly. Get details.
